Kids with black teeth: what parents should know

Black teeth in kids are more common than many parents think, and they call for a prompt assessment by a dentist.

Black teeth can be a sign of advanced cavities which, left untreated, can turn into a rotten tooth.

Sensitivity to hot and cold, pain when eating or drinking, and bad breath can also signal a developing problem. It is best not to wait for pain: see a dentist at the first signs.

Why do children's teeth turn black? The main causes

Tooth decay: the most common cause

Tooth decay is the main cause of black teeth in children. Mouth bacteria break down sugars into acids that damage the enamel; over time, the bacteria go deeper into the tooth and the decay shows as a dark or black color. Brushing that is not thorough or regular enough leaves plaque on the teeth, and candy, sweet drinks and starchy foods "feed" the bacteria and speed up decay. Stages and treatment at each stage: tooth decay in children. Black teeth are therefore not just a cosmetic issue but a problem that needs a dental check.

A black tooth without decay: surface stains, injury and iron supplements

Not every dark tooth is a decayed tooth. Food coloring, certain medicines and enamel defects present from birth can also change the color of a tooth, and three other causes are common in children:

Black stain (extrinsic dark spots)

Dark dots or lines on the crown, along the gum line, on a tooth that is otherwise sound. This is a surface stain that does not damage the tooth and comes off with a professional cleaning; in a systematic review, about 9% of children in the studies had this kind of stain, and they did not have more decay (Borrell-Garcia, 2025). For dark marks on adult teeth, see black stains on teeth.

A baby tooth that turned dark after a knock

A baby tooth that darkens after a knock should be checked by the dentist, even if it does not hurt.

A gray or black color can be a sign of damage to the pulp (nerve). According to the AAPD, follow-up after an injury is essential to catch complications, and a small gum boil above the tooth (sinus tract), alone or with a color change, calls for timely treatment to protect the developing permanent tooth (AAPD). What to do right after the injury: emergency dentist and dental first aid.

Iron supplements

Liquid iron supplements can stain the teeth. MedlinePlus advises mixing the liquid with water or fruit juice to avoid staining (MedlinePlus). The pediatrician decides on the supplement and how to give it, so do not stop it without asking; the dentist can tell this kind of stain from decay.

Can black teeth in kids go back to their natural color?

It depends on the cause: light surface stains can be removed with a professional cleaning, early decay can be stopped, and advanced decay needs a restoration.

01

Deeper decay

When tooth decay has gone deeper and made a hole in the tooth, treatment may involve tooth-colored composite fillings suited to children or, in complex cases, pulp treatment (pulpotomy) to keep the tooth alive.

02

Baby teeth

The child's age and the number and type of teeth (see how many teeth children have) influence the decision. When a baby tooth is due to fall out soon, the dentist may choose a less invasive approach; baby teeth still need care to prevent pain and infection.

Why black teeth in kids should not be ignored

Ignoring black teeth in children can lead to significant health problems. Untreated decay progresses quickly and can cause inflammation, infection and even a dental abscess that needs urgent treatment.

Patient holding her cheek because of a toothache

Baby teeth hold the space the permanent teeth will need. Losing them early because of untreated decay can disturb the bite and lead to more complex orthodontic treatment later. Infection in a baby tooth can also damage the permanent tooth developing beneath it.

Tooth pain can affect a child's eating, sleep, concentration and learning. According to the CDC, poor oral health can affect children's quality of life and how they do at school.

How to prevent black teeth in kids

Daily oral hygiene is the first line of defense against black teeth.

Good cleaning habits should start with the first tooth, using a toothbrush and a small amount of fluoride toothpaste suited to the child's age. From the first tooth until age 3, use a smear the size of a grain of rice; from 3 to 6, a pea-size amount, with an adult supervising, and the child spits instead of swallowing (ADA). Brush twice a day for two minutes, reaching every part of the mouth.

6months

Usual interval between check-ups, or as the child's risk requires

Diet matters just as much: limit sugary foods and drinks, avoid juice or sweet drinks in a bottle or sippy cup during the day or night, and prefer set meals to constant snacking, so saliva can neutralize acids.

Routine dental check-up at the clinic

Regular visits to a children's dentist are part of prevention: the first visit is recommended when the first tooth comes in, and no later than the first birthday, then usually every six months or at another interval based on the child's risk (AAPD). Sealants on the back teeth and fluoride treatments add protection: according to the CDC, sealants prevent about 80% of cavities in the back teeth. For a sudden injury or pain, learn the basics of dental first aid.

What happens at the dental visit?

The first visit includes a full assessment of the teeth, including identifying the cause of the darkening. The dentist then builds a treatment plan adapted to your child's age, ability to cooperate and the severity of the problem, with advice on preventing the problem from coming back.

Treatment is gentle, under local anesthesia when needed, and the team uses distraction and behavior techniques to help children feel at ease. Consultations can be held in English at 12 HaTnufa Street in south Jerusalem; parents receive a written treatment plan before treatment starts.

Black teeth in kids: frequently asked questions

The main reason children's teeth turn black is tooth decay: mouth bacteria break down sugars into acids that damage the enamel, and as decay spreads into the tooth it shows up as a dark or black color. Less common causes include surface stains from foods, drinks or certain medicines, enamel defects present from birth, and an injury that damages the nerve of a baby tooth.

Whether black teeth in kids can go back to their natural color depends on the cause and severity. Light surface stains from foods or dark drinks can often be removed with a professional cleaning. Early decay can sometimes be stopped with fluoride, but when decay is advanced the tooth needs to be restored with a filling or a crown.

Besides a darkening tooth, sensitivity to hot and cold, pain when eating or drinking, and bad breath can point to a developing dental problem. It is best not to wait for pain: a child with dark spots on the teeth should see a dentist at the first signs, when treatment is simpler.

A black baby tooth that is due to fall out soon is sometimes treated with a less invasive approach, as decided by the dentist. Even so, baby teeth still need professional care to prevent pain, infection and problems with the development of the permanent teeth.

From the first tooth until age 3, children should use a smear of fluoride toothpaste the size of a grain of rice; from age 3 to 6, a pea-size amount, with an adult supervising, and the child spits out the toothpaste instead of swallowing it (ADA). Brush twice a day for about two minutes, reaching every part of the mouth.
