Dental implants: a fixed solution for a missing tooth

At Dr Haim Aseriel's dental clinic in Jerusalem, treatment follows a clear principle: a natural tooth that can be saved is preferable to any implant. When a tooth cannot be kept, however, a dental implant offers a replacement that combines function with a natural look. The consultation can be held entirely in English, and you leave with a written treatment plan.

How does a tooth implant work?

Dental implants are small titanium screws placed inside the jawbone to replace the root of a lost tooth.

The bonding between the implant and the bone is called osseointegration, and it usually takes three to six months. Once the implant is stable, a natural-looking dental crown is attached to it. Unlike a fixed bridge, a dental implant does not require reshaping the neighboring teeth, and it helps preserve the jawbone where the tooth is missing.

Who are dental implants for?

Many patients come to the clinic after a tooth extraction, an accident, or the loss of a tooth to advanced gum disease. In every case, whether an implant is suitable is assessed carefully before any decision is made.

Dental X-rays on a screen used to plan an implant

A tooth implant is suitable for most healthy adults whose bone growth is complete, and there are also solutions for people who lack bone.

Patients with active gum disease, uncontrolled diabetes or heavy smoking need a specific assessment and preparation before surgery.

When bone volume is insufficient, short dental implants can be considered in suitable cases. However, according to a meta-analysis cited in a large registry study, implants shorter than 10 mm were associated with a higher risk of early failure (Tobias et al., Journal of Functional Biomaterials, 2025), which is why precise planning is essential.

Types of dental implants and techniques

The technique is chosen according to the number of missing teeth, the amount of bone and the timing of the extraction:

  • Same-day dental implants: an implant placed right after the extraction, sometimes with a temporary crown the same day, when the bone and stability allow it.
  • Full mouth dental implants: a complete arch restored with a fixed bridge supported by several implants, when most or all teeth are missing.
  • All-on-4 dental implants: a fixed full-arch bridge on just four implants, two of them tilted to make use of the available bone.
  • Basal implants: implants anchored in the deep, dense layers of the jawbone, an option when the alveolar bone is thin.
  • Zygomatic implants: long implants anchored in the cheekbone, when the upper jaw does not have enough bone for standard implants.
  • Implant uncovering: a short procedure under local anesthesia to expose the head of an implant left under the gum, before the crown is attached.

Why replace a missing tooth?

Losing even one tooth can start a chain of problems: neighboring teeth drift, the bone keeps shrinking, and chewing becomes less efficient.

Compared with removable dentures, a dental implant gives full stability and the feel of a natural tooth, with no risk of slipping, including as part of a full mouth rehabilitation.

The dental implant process, step by step

The tooth implant process follows defined stages, from the suitability assessment to the final restoration.

1. Consultation and 3D imaging

The quantity and quality of the bone are assessed with 3D imaging taken at the clinic (a cone beam CT scan, or CBCT, on a Planmeca ProMax 3D), and the implant is planned, with a 3D-printed surgical guide when needed.

2. Implant surgery

The titanium implant (MIS implants) is placed under local anesthesia. Surgery is performed at the clinic together with Dr Yves Weinberg, head of the Department of Oral and Maxillofacial Surgery at Barzilai Medical Center.

3. Healing (osseointegration)

A waiting period of a few months, usually 3 to 6, while the implant bonds with the bone.

4. Implant uncovering

When the implant was left under the gum, a short uncovering procedure exposes its head before the crown is attached.

5. Placing the crown

A metal-free zirconia or E.max crown is fitted on the implant.

How to prepare for dental implant surgery

Proper preparation before dental implant surgery improves the chances of success and makes recovery easier.

Dentist examining a patient before implant surgery

Before any surgery, gum health is assessed thoroughly and, when needed, gum treatment is carried out first.

  • Active gum disease (periodontitis) is a recognized risk factor for inflammation around implants and implant failure.
  • Smoking increases the risk of implant failure, so smokers are asked to stop or cut down around the surgery, following the dentist's instructions.
  • Patients who take or have taken bone-strengthening medications, such as bisphosphonates, must say so: these drugs are linked to a risk of jaw osteonecrosis (MRONJ) after oral surgery (Ruggiero et al., Journal of Oral and Maxillofacial Surgery, 2022).
  • When indicated, for example in a sinus lift or a bone graft, PRF is used: a concentrate prepared from the patient's own blood in the clinic's centrifuge.

Aftercare: how to protect your dental implants

Keeping dental implants healthy for years requires careful daily hygiene and regular check-ups with the dentist.

For the first two weeks, avoid hard foods and smoking. Daily cleaning with floss designed for implants and a soft toothbrush is essential. An implant that is not cleaned properly can develop inflammation around it (peri-implantitis).

What to expect after dental implant surgery

In most cases, discomfort after dental implant surgery is moderate; ask your dentist or pharmacist which pain reliever and dose are right for you.

A single implant

After a single implant, many patients go back to their usual activities within a few days, with mild swelling at most.

Case from the clinic

A 45-year-old man who had a single implant placed went back to work two days after surgery, with only mild swelling.

More complex cases

In more complex cases, such as full mouth dental implants, recovery can take a week to ten days.

96.0%

  • survival rate (up to 6 years of follow-up)

94.3%

  • success rate

A study of 108 patients followed for up to six years found a survival rate of 96.0% and a success rate of 94.3% (Charyeva et al., Swedish Dental Journal, 2012).

Complications and side effects

Some side effects are expected after any surgery, but it is important to tell a normal reaction from a warning sign that needs checking.

  • Swelling: normal local swelling, peaking on the second or third day.
  • Moderate pain: common in the first days; the type of pain reliever and the dose are decided with the dentist or pharmacist.
  • Light bleeding: normal in the first hours; it stops with gentle pressure.
  • Numbness: numbness that lasts after the anesthesia has worn off is not expected and must be reported to the dentist right away.

Over the long term, the most common complication is inflammation around the implant: according to a systematic review, inflammation of the soft tissue around the implant (peri-implant mucositis) affected about 43% of patients on average, and peri-implantitis, which involves bone loss, about 22% (Derks & Tomasi, Journal of Clinical Periodontology, 2015). Daily hygiene and regular check-ups help prevent it.

Severe pain that gets worse after the third day, fever, spreading swelling or a moving implant are signs that require contacting the dentist immediately. Fever with swelling spreading toward the eye or the neck, or difficulty swallowing or breathing, require going to the emergency room right away.

Is dental implant surgery done under general anesthesia?

Dental implant surgery is usually performed under local anesthesia and does not require general anesthesia in most cases.

Local anesthesia is there to prevent pain during the surgery, although you may feel some pressure. For patients with significant dental anxiety, light oral sedation can be added, by prescription. Unlike surgery under general anesthesia, which requires fasting and anesthetic monitoring, implant surgery under local anesthesia usually lets you go home right afterwards. All-on-4 dental implants can also be placed under local anesthesia, even for a full arch. Complex cases, such as zygomatic and basal implants, are also treated at the clinic under local anesthesia, together with an oral and maxillofacial surgeon.

Implant options when bone is limited

Being told that you "don't have enough bone" does not always rule out implants. Depending on the anatomy, the options include a dental bone graft, a sinus lift in the upper back jaw, short implants, tilted implants as in the All-on-4 concept, and, for selected cases, basal implants or zygomatic implants. The level of scientific evidence differs between these options, and it is explained at the consultation.

Dental implant cost: what makes up the price?

There is no single price for dental implants: the cost depends on your individual treatment plan. The main factors are the number of implants, any preparatory procedures such as a bone graft or a sinus lift, the type of crown (zirconia or E.max), 3D imaging and planning, and a temporary restoration while the implant heals. After the examination and imaging you receive a detailed written quote covering every stage of treatment. More details on what is included: dental implant cost.

Dental implants in Jerusalem at Dr Haim Aseriel's clinic: consultation, planning and long-term follow-up

A meta-analysis of implant survival over 20 years of follow-up stresses that regular check-ups remain important many years after implant placement (Kupka et al., Clinical Oral Investigations, 2024).

Case from the clinic

A man who had comprehensive oral rehabilitation before his implant came back for a check-up two years later: the implant was stable and he had no complaints.

Dr Haim Aseriel, a US-trained dentist (DDS, NYU College of Dentistry), plans each implant treatment with the clinic's dental team and explains every step in English. The clinic is at 12 HaTnufa Street in Talpiot, south Jerusalem, open Sun 8:00–19:00 · Mon 9:00–19:00 · Tue–Wed 9:00–21:00 · Thu 8:30–21:00 · Fri 8:00–14:00.

At the end of the consultation you receive a detailed written quote, and the clinic provides itemized receipts that you can submit to your insurer (US dental plan or Israeli supplementary insurance); reimbursement depends on your policy. You can also read patient reviews.

Dental implants: frequently asked questions

A dental implant is a small titanium screw placed in the jawbone to replace the root of a missing tooth; a crown is then attached to it. The bonding of the implant with the bone is called osseointegration, and it usually takes a few months (most often three to six) before the permanent crown is attached.

Dental implants are suitable for most healthy adults whose jaw growth is complete, and there are solutions for people who lack bone, such as bone grafting or short implants. Active gum disease, uncontrolled diabetes and heavy smoking call for a specific assessment and preparation before implant surgery.

Dental implants usually last many years: according to a meta-analysis of follow-up studies, about 96% of dental implants survived 10 years. Daily oral hygiene and regular check-ups with the dentist help prevent inflammation around the implant and protect the result.

After dental implant surgery, swelling and moderate discomfort are common for the first few days. Ask your dentist or pharmacist which pain reliever and dose are right for you. Severe pain that gets worse after the third day, fever or a moving implant require contacting the dentist right away.

Partly. According to the Israeli Ministry of Health, from age 72 the health basket (sal habriut) includes preventive and restorative dental care and dentures, including up to 4 implants to stabilize a complete lower denture. The entitlement is used through your health fund (kupat cholim), so check the details with it before treatment.
