Dental examination before fitting an NTI-TSS device

What is the NTI-TSS device?

The NTI-TSS night guard (Nociceptive Trigeminal Inhibition Tension Suppression System) is a partial dental appliance designed for problems related to the chewing muscles. Unlike a traditional night guard that covers all the teeth, the NTI dental appliance is a small device fitted only to the upper or lower front teeth. It prevents the back teeth from closing fully, which reduces the force the chewing muscles can generate and the load on the jaw joints.

The NTI-TSS device was cleared by the U.S. FDA in 2001 (510(k) K010876) as a device for the prevention of migraine and tension-type headaches. This clearance is not proof of effectiveness, and controlled studies on it are few.

The device is made of durable acrylic and adjusted to each patient's mouth. According to a systematic review, it reduced chewing muscle activity in studies on bruxism, but to avoid unwanted effects it should be chosen only for patients who reliably attend their follow-up visits (Stapelmann & Türp, 2008).

Criterion NTI-TSS device Full-coverage night guard
CoverageSmall partial device on the front teeth onlyCovers all the teeth of one arch
PrincipleReduces muscle activity by preventing full closure of the jawsSpreads the forces over the whole arch and protects against wear
IndicationFDA-cleared for the prevention of migraine and tension-type headaches; research evidence is limitedProtects the teeth from wear caused by grinding or clenching
Follow-upRegular checks of the device and of the bite are essentialPeriodic checks of the guard
Risks and limitsBite changes if worn long-term without follow-up; rare risk of swallowing or inhaling the deviceProtects the teeth but does not cure bruxism itself

How might an NTI night guard help with headaches?

Migraines and tension-type headaches are sometimes associated with overactive chewing muscles, in particular with clenching and night grinding (bruxism). Grinding or clenching during sleep creates strong muscle tension around the temples and the jaw, an area served by the trigeminal nerve.

Patient holding her jaw because of pain

According to the proposed mechanism, the NTI guard keeps the back teeth from meeting, which limits the force the temporalis and masseter muscles can generate. It is intended to reduce this muscle activity and the related headaches; the research evidence is limited (Stapelmann & Türp, 2008).

Some patients report fewer headaches and better sleep, but the response varies from one patient to another. The FDA clearance does not concern teeth grinding: to protect the teeth from wear, a full-coverage night guard is the usual option.

Who may benefit from an NTI-TSS device, and who should avoid it?

An NTI-TSS device may be considered for patients with migraines or tension-type headaches associated with night clenching or grinding, especially when there is a link between the jaw activity and the headaches. The decision is made only after an examination.

Jaw joint problems

The device is sometimes considered for patients with jaw joint disorders (TMD), such as jaw pain, difficulty opening the mouth or clicking in the joint. Here too, the evidence on its effectiveness is limited.

Risks and contraindications

Because it covers only the front teeth, an NTI guard worn for a long time without regular dental follow-up can change the bite, and there is a rare risk of swallowing or inhaling the device. It is generally not suitable for patients with advanced gum disease, loose teeth or teeth that do not allow a good grip, or for children and teenagers whose teeth are still developing.

The assessment includes a clinical examination, dental X-rays when indicated, and detailed questions about your symptoms and medical history. This matters because the NTI-TSS device is more likely to be relevant when headaches are linked to overactive chewing muscles, and less so when migraines have other causes, such as hormonal or neurological factors, which require an assessment by your physician.

The NTI-TSS treatment process, step by step

Treatment with an NTI-TSS device is not just the appliance itself: it includes diagnosis, precise fitting, follow-up and adjustments.

01

Consultation

A clinical assessment of the chewing system determines whether an NTI-TSS device suits you or whether a full-coverage night guard is more appropriate.

02

Impression and fitting

An accurate impression of the teeth is taken; the device is made by Glidewell (USA) and adjusted in the mouth. You receive instructions on how to wear and clean it.

03

Follow-up visits

Regular follow-up visits are essential: the dentist checks the fit of the device and your bite in order to detect any change early.

NTI-TSS night guard cost

The cost of an NTI-TSS night guard depends on the complexity of the case, any preliminary examinations and the degree of customization required. The clinic does not publish a price list: after the examination you receive a detailed written estimate covering diagnosis, the device, fitting and follow-up, and itemized receipts you can submit to your insurer (coverage depends on your policy).

NTI-TSS night guard: frequently asked questions

The NTI-TSS (Nociceptive Trigeminal Inhibition Tension Suppression System) is a small, partial dental appliance that covers only the upper or lower front teeth and keeps the back teeth from touching. It was cleared by the U.S. Food and Drug Administration in 2001 (510(k) K010876) as a device for the prevention of migraine and tension-type headaches.

No. The FDA clearance of the NTI-TSS device concerns its use to help prevent migraine and tension-type headaches; it is not proof of effectiveness, and controlled studies are few. Some patients report fewer headaches, but the response varies, and migraines always call for an assessment by a physician.

Because an NTI dental appliance covers only the front teeth, wearing it for a long time without regular dental follow-up can change the bite. There is also a rare risk of swallowing or inhaling the device. For these reasons it is suitable only for patients who attend their follow-up visits.

An NTI-TSS guard is generally not suitable for patients with advanced gum disease, loose teeth or a tooth structure that does not allow the device to grip well, or for children and teenagers whose teeth and jaws are still developing.

The cost of an NTI-TSS night guard depends on the complexity of the case, any preliminary examinations and the degree of customization. It includes not only making the device but also diagnosis, precise fitting, follow-up visits and adjustments. A written estimate is given after the examination.
