What are same day dental implants?
Same day dental implants make it possible, in suitable cases, to place an implant right after the extraction and to leave the clinic with a temporary tooth on the implant the same day.
The traditional protocol developed by Per-Ingvar Brånemark required waiting 5 to 6 months after a tooth extraction before placing the implant (Ebenezer et al., Journal of Pharmacy and Bioallied Sciences, 2015). Current protocols allow the overall treatment time to be shortened considerably for suitable patients. Same-day implants are placed at Dr Haim Aseriel's dental clinic in Jerusalem under local anesthesia, using MIS implants, after 3D (CBCT) planning.
Benefits of same-day implants
Immediate placement can save a surgical step and shorten treatment compared with delayed placement; its effect on bone volume and on the aesthetic result depends mainly on the thickness of the bone wall and of the gum.
According to a meta-analysis, the survival rate of implants placed immediately after extraction (97.4%) did not differ significantly from that of implants placed after healing (97.5%) (Patel et al., Dentistry Journal, 2023). These data show that immediate dental implants, performed under the right conditions, are an established treatment option.
Candidate check: the conditions for a same-day implant
Not every patient is suitable for a same-day implant (for example, to replace a broken tooth that cannot be restored), and choosing the right timing largely determines success.
- Front bone wall thickness: an intact wall more than 1 mm thick, for a good aesthetic result (Peitsinis et al., Journal of Clinical Medicine, 2025).
- Enough bone beneath the socket: at least 3 mm of bone beyond the tip of the tooth socket for primary stability (Gupta et al., Contemporary Clinical Dentistry, 2019).
- No acute infection: the implant site must be free of active inflammation.
- Primary stability: a sufficient ISQ value before the temporary crown is attached.
- Healthy soft tissue: enough gum tissue for a good seal and a natural look.
Patients with uncontrolled diabetes, heavy smoking, active osteoporosis or insufficient bone are not good candidates. Less than 3 mm of bone beyond the tip of the socket is a contraindication to immediate placement (Gupta et al., Contemporary Clinical Dentistry, 2019). In such cases, a sinus lift or a dental bone graft may be a necessary preparatory step before the implant.
According to a literature review citing a study by Kan and colleagues, the rate of instability was only 1.1% for tapered implants versus 20.5% for cylindrical implants; tapered implants are therefore generally preferred for immediate placement (Huang et al., Journal of Dental Sciences, 2023).
Immediate, early or delayed: the implant timing protocols
The scientific literature distinguishes three main protocols: immediate placement (Type I), early placement (Type II) and delayed placement (Type III/IV).
| Protocol | When after the extraction | Conditions | When it is chosen |
|---|---|---|---|
| Immediate placement (Type I) | Right after the extraction, in the existing socket | Intact bone wall, enough bone beyond the tip of the socket, no acute infection and primary stability | When all conditions are met and the aim is to shorten treatment and save a surgical step |
| Early placement (Type II) | Usually 4 to 8 weeks, after initial soft-tissue healing | Gum healed over the socket | When immediate placement is not possible but full bone healing is not needed |
| Delayed placement (Type III/IV) | 3 to 4 months in the lower jaw, 6 to 8 months in the upper jaw | Complete bone healing | When there is infection, thin bone or a need for bone grafting first |
Delayed placement timings follow the Adell protocol (Huang et al., Journal of Dental Sciences, 2023).
In practice, the choice of protocol depends on the anatomy, the quality of the bone and the patient's general health, which is why precise planning comes before any decision, as part of a full mouth rehabilitation approach when several teeth are involved.

Same-day implant procedure: how it works
A same-day implant procedure includes several steps carried out at the same appointment: planning, extraction, implant placement and, sometimes, an immediate temporary crown.
The first step is detailed planning based on 3D imaging (CT) and a clinical exam. After the extraction, the implant is placed and its primary stability is measured; the type of implant and the protocol are chosen according to these findings. For example, in a front tooth with healthy bone, a tapered implant can be placed and a temporary crown attached the same day, whereas with poor bone the implant must first heal, sometimes followed by implant uncovering, before the permanent crown is attached.

Drawbacks and risks of same-day implants
- Not suitable in every situation: active infection at the extraction site or thin bone usually call for early or delayed placement, sometimes with bone grafting.
- Primary stability is required: if the implant is not stable enough in the bone when it is placed, no temporary crown is attached that day.
- Soft diet while it heals: until the implant has fused with the bone, stick to soft foods and follow your dentist's chewing instructions.
- A slightly higher risk of early failure: in a meta-analysis, slightly more immediate implants failed, and in some studies their survival rate was 90% to 95%, compared with more than 95% for delayed placement (Patel et al., Dentistry Journal, 2023).
How much do same day dental implants cost?
The cost of a same day implant depends on several factors: the number of implants, the type of crown, any associated bone procedures and the complexity of the individual treatment.
There is no single price for same-day dental implants, because the treatment plan is tailored to each patient's situation.
After a thorough clinical examination with the necessary imaging, you receive a detailed written quote covering every stage of treatment, and itemized receipts you can submit to your insurer (coverage depends on your policy). More on the factors that affect the price: dental implant cost.
Same-day vs conventional implants: the differences
The main difference between immediate and conventional implant placement is the timing of the implant relative to the extraction, not the quality of the implant itself.
With conventional placement, the bone is left to heal fully first (3 to 8 months depending on the area); a same-day implant uses the existing tooth socket to stabilize the implant immediately. After a temporary crown (see also same-day crowns), osseointegration continues before the permanent dental crown is fitted. In practice, immediate placement reduces the number of surgical steps and shortens overall treatment time, but it requires more precise planning and careful patient selection.
Same-day teeth on implants with the All-on-4 concept
The All-on-4 concept restores a complete arch on just four implants, with the two back implants tilted at up to about 45° to make use of the available bone.
The All-on-4 approach was developed by Paulo Maló (2003) and is particularly suited to patients with extensive tooth loss and limited bone.
Two tilted back implants
The two tilted back implants bypass areas of thin bone and allow a full temporary bridge to be attached on the day of surgery. Compared with traditional implant rehabilitation, which sometimes uses 6 to 8 implants per arch, this approach can reduce the number of surgeries, the overall treatment time and the cost.
Why tilt the implants?
Tilting the back implants allows longer implants to be placed in the existing bone and positioned further back, so they can support the bridge even when bone volume is limited.
Recovery after same-day implants
Many patients think that same-day dental implants also mean same-day healing. In fact, the visible part is finished on the day of treatment, but osseointegration takes months.
After immediate placement, the implant usually takes 3 to 6 months to bond with the bone, depending on the area of the mouth and the quality of the bone.
During healing you wear a functional temporary crown that allows normal eating (soft foods first, as the dentist advises) and speaking. Once integration is confirmed, the temporary crown is replaced with a permanent crown in zirconia or E.max. The clinic follows you from the start of treatment to the final crown.
Same-day dental implants in Jerusalem: consultation and suitability check at Dr Haim Aseriel's clinic
Dr Haim Aseriel, a US-trained dentist (DDS, NYU College of Dentistry), bases every treatment plan on a thorough examination and on the clinic's conservative philosophy: saving natural teeth first, and using implants only when there is no reasonable conservative alternative. The consultation can be held in English at the clinic, 12 HaTnufa Street in Talpiot, south Jerusalem. You can also read patient reviews.
Same-day dental implants: frequently asked questions
Same day dental implants are possible for some patients, but not for everyone: success depends on anatomical and clinical conditions that must be checked beforehand, such as the thickness of the bone wall, the absence of active infection and the primary stability of the implant. Uncontrolled diabetes, heavy smoking, active osteoporosis or insufficient bone make a patient a less suitable candidate.
The main difference between immediate and conventional implant placement is the timing of the implant relative to the extraction, not the quality of the implant itself. According to a meta-analysis, implants placed immediately and implants placed after healing have similar survival rates (about 97%). Immediate placement can save a surgical step and shorten treatment, but it requires precise planning and careful patient selection.
The cost of same-day dental implants is not uniform: it depends on the number of implants, the type of crown, the need for associated bone procedures and the complexity of the case. A detailed written quote is given after a clinical examination and X-rays.
After a same-day implant, the visible part of the treatment is completed on the day of surgery, but osseointegration (the bonding of the implant with the bone) usually takes three to six months, depending on the area of the mouth and the quality of the bone. Only then is the temporary crown replaced with a permanent one.
